Dr. Robert Fogel has been in practice for 26 years serving the greater NY Metropolitan Area with high quality affordable and gentle chiropractic health. In addition to earning a Doctorate of Chiropractic from NY Chiropractic College in 1980, Dr. Fogel received his BA from SUNY Binghamton and completed a Masters Degree from Columbia University. Dr. Fogel has continued his postdoctoral studies emphasizing hands on manual therapies targeting the myofascial system and associated stress related tension. BACK PAIN AND NECK PAIN

Back pain is the most common condition treated by chiropractic therapy.

If you are experiencing back pain that continues for more than 7 days you need to make an appointment for a chiropractic evaluation. A simple strain that continues untreated without correction can turn into a serious chronic problem.

After a complete orthopedic, neurological and chiropractic examination Dr Fogel will diagnose and determine whether your back pain is of mechanical (muscle, ligament, tendon or joint origin) or whether your problem is arthritic in nature.


Q: How did I get this back pain?

Most mechanical back problems start with a mild strain associated with a fall, sudden twist or heavy lift from a weak unsupported posture. Although the original incident may have occurred years ago a new stress/strain or a repetitive minor strain may provoke a new bout of pain.

Repetitive strains are numerous and they include house chores, painting, mopping, new exercise programs, reaching upward to a shelf, prolonged sitting in front of computer monitor turned to either right or left.

The cervical spine is the upper part of the spinal column between the head and shoulders and is composed of seven of freely movable spinal joints. The neck is subject to a number of common pain syndromes involving both mechanical and arthritic causes. Neck strain can arise from improperly designed work environments such as computer screens set too far to the right or left of center, holding a telephone between the ear and shoulder, sleeping in the presence of a fan or cold draft and sleeping on a couch.

Whiplash or trauma stemming from a motor vehicle accident can cause micro tearing of muscle, tendon and ligamentous structures, resulting in mild to severe neck pain. 

Tension, emotional stress and physical illness with prolonged bed rest all can cause sustained neuromuscular contraction of the neck.

HEADACHES

Tension Headaches are often associated with mild neck pain or stiffness. A careful examination and history which reveals a connection between your longstanding or chronic neck pain points toward a very high probability of success in treatment.

Research has demonstrated that 80% of all headaches are cervicogenic (caused by dysfunction of the neck spinal joints and or musculature). Wearing new bifocals, prolonged viewing of a computer monitor, reading or watching TV in bed, whiplash etc,  have all been associated with neck-related headache. The upper neck is particularly vulnerable to headache causing symptoms including dizziness and mild visual disturbances. Dr. Fogel has had great success with these types of cases.

SCIATICA

Sciatica is another common condition treated at our office. Sciatica refers to a syndrome of low back, leg, hip and buttock pain.

Sciatica has several possible causes and degrees of complexity. The good news is that more than 50% of sciatic cases seen here will be treated successfully.
We provide a comprehensive evaluation to determine the cause of your sciatica. Dr. Fogel will let you know whether your problem is a herniated disc, a pyriformis muscle spasm or an osteoarthritic spine or hip. 

A more serious herniated disc is accompanied by muscle weakness, numbness and reflex change. In this situation you will be referred to an excellent spinal surgeon.

DISCS

Discs are fibro-cartilagenous pads that link the bony spinal blocks or vertebra of the spinal column. Inside each disc is a soft spongy spherical core which allows the vertebral plates above and below to pivot or tilt. This core is called the nucleus pulposus.
The vertebra  above and below the sandwiched disc forms a functional unit that performs three essential and very different functions:  
1.    Discs  absorb the weight/pressure of the upper torso and head as the body’s main structural column.2. Discs allow the otherwise bony spinal column to flex, laterally bend and rotate.
3. The spinal column which is  composed of multiple vertebra and disc units, houses and protects the delicate nerve bundle- the spinal cord and its spinal nerve branches, which exit the spine at each disc level to control and communicate with the organs, muscles and blood vessels.
The disc vertebral unit has a very special relationship to the spinal nerves as they exit the spinal column and perform their many functions of signaling, control and regulation.

There are three basic types of disc problems:

1. Leg pain disc caused by a ruptured or herniated nucleus propulsus moving outside the confines of the surrounding disc and compressing a nerve or nerve covering.

2. Back pain disc caused by an internally disrupted disc without rupture or herniation and yet disrupting the pain sensitive fibres surrounding the nucleus or central disc. This creates back pain and occasional buttock and leg pain.

3. A mixed pattern can arise with small contained disc herniations with variable amounts of leg, buttock and back pain although without compression of a nerve.

OSTEOARTHRITIS

Osteoarthritis (OA) is the most common joint disease of the spine and extremities (shoulders, hips, knees ). The smooth gliding surfaces at the ends of bone referred to as articular cartilage loses its mechanical resistance, elasticity and smoothness, and is consequently worn out by the movements of the joint. This process called “reactive bone remodeling”, will cause osteophytes (think of calcium barnacles growing along the end surface of bones ), microfractures, subchondral eburnation and pseudocysts, and exposure of the articular end of the bone. The consequent roughness of the articular cartilage surfaces elicits secondary inflammatory reactions of the synovial membrane and bone. Unlike rheumatoid arthritis and other inflammatory joint diseases, the inflammatory component of OA is relatively mild.Clinical manifestations of OA are joint pain, stiffness in the morning or after rest, pain at night, limited joint motion and/or joint deformity. Joint pain in OA may also be caused by stretching of the joint capsule or ligaments, bone irritation due to osteophyte formation, microfractures, bone swelling, or muscle spasm.
NSAIDs ( Ibuprofen, Advil, Motrin etc.) are the most widely prescribed drugs to reduce joint pain and stiffness. Since the inflammatory component of OA is usually minimal, the need for the anti-inflammatory effect of NSAIDs in this condition is controversial. Occasionally severe side effects including gastrointestinal bleeding, hypertension, congestive heart failure, hyperkalemia, and renal insufficiency make these a poor choice for many patients. Any advantage gained from NSAIDS will come from the pain relieving( analgesic ) quality of these drugs.
Recently medical doctors are prescribing analgesics like paracetamol  for OA of the knees. However, the long term use of paracetamol may lead to liver and kidney damage. These disadvantages clearly calls for the use of a less toxic pain managing therapy for OA such as chiropractic and massage.

Whiplash, Auto or Motor Vehicle Accident Spinal Soft Tissue Injuries


Injuries sustained while traveling at high rates of speed is a relatively new type of health problem never before seen on our planet till the invention of the auto at the beginning of the twentieth century.
If you fell off a high cliff and hit the bottom at 35 MPH… chances are you didn't survive.


Seriously though, injuries sustained in motor vehicle accidents (MVA) are a public health problem. The costs are great: both economically, and personally. Not only are many people killed, yet many of those who do survive are left with lifelong impairments and disability of varied degrees. The patient(s) who continue to live with chronic varied degrees of  pain, impairment and disability is a controversial topic unfortunately, due to the involvement of insurance companies, lawyers and money and doctors who fail to appreciate the nature of chronic pain.

There are many of you who continue to suffer with pain and disability as a result of auto accidents long after the event.


The physics of forces that come to bear on the occupants of colliding vehicles weighing approximately 2000 to 3500 pounds are tremendous. A head-on collision can create 5500lbs of force on a spine, an unsupported head and neck.


In this discussion I will focus on the soft tissue injuries most likely to result from the motor vehicle accident.

Strain/Sprain


Muscles are soft tissues (the meat of animals, fowl and fish we eat) that are attached at their tapered ends to tendons and ultimately bone and thus and move the skeletal system.
Muscles are like cloth in that they are both composed of tiny fibers, which when subjected to traumatic force may stretch increasingly and to the point they eventually fail and thus tear.


A muscle tear may be mild, moderate or severe. Symptoms will reflect the degree of injury, of course. Pain, spasm, swelling, hemorrhage and loss of active range of motion may be associated with various degrees of injury.

Similarly, ligaments which strap bone to bone and thus hold our skeletal frame together, are subject to forces which my strain and eventually cause tearing of structure,

Spinal discs are particularly vulnerable to rotary forces combined with compression or bending
Discs may tear, bulge, protrude and herniate. See section on discs for additional information including anatomy.

WORK INJURIES

If you are injured at work while performing your job duties New York State Workers Compensation Law provides for no out of pocket medical treatment. 
If you have a work related injury and need treatment it is your responsibility to make sure your employer has filed the proper C2 accident report and that it is filed with the State.

Dr Fogel has been treating injured workers for over twenty-two years and is committed to your rapid, healthy and safe return to work. If you have a workers compensation hearing you can rely on him to be there and testify on your behalf. He will provide you a work absence note, detailing the nature and diagnosis of your work- related injury and probable return to work date.
When you are ready to return to work, Dr Fogel will write for you a return to full duties authorization letter. If you are having problems with the workers compensation system, you will be referred to an excellent workers compensation attorney.
